CoQ10 and Statins: What the Research Shows
Statins lower the amount of CoQ10 in your blood; a review of 12 trials found that clearly. Whether extra CoQ10 eases statin muscle aches is still unsettled. One review found no benefit, a later one found less pain, and the trials disagreed a lot. CoQ10 can sit beside a statin, never in its place, and only after your doctor agrees.

Why do people talk about CoQ10 and statins together?
Because statins and CoQ10 share a production line in the body. Statins slow the liver's cholesterol factory, and the same pathway also makes CoQ10.
Statins are prescription drugs that lower LDL cholesterol by slowing how much cholesterol your liver makes (MedlinePlus - Statins). They block an early step in a chain of chemical reactions. Cholesterol comes out of one end of that chain. CoQ10, a compound your cells use to make energy, comes out of another branch.
So when a statin turns down the chain, it can turn down CoQ10 too. That simple idea is why many people on statins ask about CoQ10 supplements. It is also why supplement ads aimed at people over 50 often mention statins.
The idea is reasonable biology. The harder question is whether taking extra CoQ10 changes how anyone feels. The research gives two different answers to two different questions, so it helps to split them apart.
02Question one
Do statins really lower CoQ10 levels?
Yes. This part of the story is well supported. A 2018 review pooled 12 randomized trials with 1,776 people and found that statins lowered CoQ10 in the blood compared with placebo (Qu 2018 - Statins lower CoQ10 levels).
The drop showed up with different kinds of statins, at low, moderate and high strength. How long people took the drug did not seem to change it (Qu 2018 - Statins lower CoQ10 levels). Memorial Sloan Kettering also notes that lovastatin, atorvastatin and simvastatin may reduce the body's own CoQ10 (MSKCC - Coenzyme Q10).
There is a catch. The trials varied a great deal in how big the drop was. Blood levels also do not tell you what is happening inside muscle cells. A lower number on a lab report is not the same as a symptom you can feel.
03Question two
Does CoQ10 help statin muscle pain?
Nobody can say for sure yet. Two meta-analyses, both pooling randomized trials, reached opposite conclusions about muscle aches.
| Detail | Banach 2015 (Banach 2015 - CoQ10 statin myopathy) | Qu 2018 (Qu 2018 JAHA - CoQ10 statin symptoms) |
|---|---|---|
| Trials pooled | 6 | 12 |
| People | 302 statin users | 575 patients |
| CoQ10 dose | Varied; no dose link found | 100-600 mg a day |
| Muscle pain | No significant change | Less reported pain, weakness, cramps and tiredness |
| Creatine kinase, a muscle blood marker | No significant change | No significant change |
| Main caution | Small trials; authors asked for bigger ones | Very high variation between trials |
Notice where they agree. Neither review found a change in creatine kinase, the blood marker doctors check for muscle damage. They split only on how people rated their own aches.
Health agencies read it cautiously too. NCCIH says the overall evidence does not support the idea that CoQ10 reduces statin muscle pain (NCCIH - Coenzyme Q10). Mayo Clinic says some research suggests it might help, but there is not enough evidence to know for sure (Mayo Clinic - Coenzyme Q10). MSKCC calls the trials inconclusive (MSKCC - Coenzyme Q10).
04Making sense of it
Why do the studies disagree?
Mostly because the trials were small and different from each other. When studies use different doses, lengths and patients, pooling them gives a blurry average.
- Muscle aches are hard to measure. Pain is scored on a scale, and people rate it differently from day to day.
- Short trials. The later review covered trials of 30 days to 3 months (Qu 2018 JAHA - CoQ10 statin symptoms). That says little about years of use.
- Wide dose range. Doses ran from 100 mg to 600 mg a day (Qu 2018 JAHA - CoQ10 statin symptoms), so there is no single tested amount to copy.
- Not every ache is the statin. Muscle damage from statins is uncommon, and aches have many other causes (MedlinePlus - Statins).
The honest summary: CoQ10 for statin aches is a reasonable thing to ask your doctor about. It is not a proven fix.

Where does a 100 mg capsule fit?
At the low end of the doses studied. Cardio Clear 7 lists 100 mg of CoQ10 per capsule, plus 80 mg shilajit extract and 10 mg PQQ, one capsule a day (Simple Promise store listing).
The muscle-symptom trials in the later review started at 100 mg and went up to 600 mg (Qu 2018 JAHA - CoQ10 statin symptoms). No trial has tested Cardio Clear 7 itself, with or without a statin. The shilajit and PQQ in the capsule were not part of any statin study we found.
The bottle's front claims are "Supports a Healthy Heart*" and "Helps Maintain Healthy Cellular Energy*". Neither says anything about statins. See the full breakdown on the CoQ10 ingredient page.
06Safety
Is it safe to take CoQ10 with a statin?
For many adults it may be, but the choice belongs with your doctor or pharmacist. CoQ10 is usually well tolerated. Mild side effects include trouble sleeping, nausea and diarrhea (NCCIH - Coenzyme Q10) (Mayo Clinic - Coenzyme Q10).
The bigger concern is other medicines. Many statin users also take a blood thinner, a blood pressure pill or diabetes medicine.
Clot and bleeding questions
CoQ10 may make warfarin work less well, and bleeding has also been reported (Mayo Clinic - Coenzyme Q10) (MSKCC - Coenzyme Q10).
Blood sugar care
CoQ10 may change how the body uses insulin (NCI PDQ - Coenzyme Q10). Your care team may want more checks.
Two-way effects
NCI notes that cholesterol and blood pressure drugs may lower CoQ10's effects (NCI PDQ - Coenzyme Q10).
Keep taking prescribed medicine; never stop or change it because of a supplement. If muscle aches bother you, tell your prescriber. A blood test or a switch to another statin may help (MedlinePlus - Statins).
07Next step
What should you ask your doctor?
Bring the bottle and a short list. A pharmacist can check the label against every medicine you take in a few minutes.

08FAQ
CoQ10 and statins: FAQ
Do statins deplete CoQ10?
Yes, statins lower CoQ10 in the blood. A review of 12 randomized trials with 1,776 people found the drop with several types and strengths of statin (Qu 2018 - Statins lower CoQ10 levels). Whether that lower level causes symptoms is a separate, unsettled question.
Should I take CoQ10 if I am on a statin?
Ask your doctor first. The research on CoQ10 for statin aches is mixed: one review found no benefit (Banach 2015 - CoQ10 statin myopathy) and another found less pain with very uneven results (Qu 2018 JAHA - CoQ10 statin symptoms). Your doctor can weigh it against your other medicines.
Which statins lower CoQ10 the most?
The 2018 review found the drop with both fat-soluble and water-soluble statins, at low, moderate and high strength (Qu 2018 - Statins lower CoQ10 levels). MSKCC names lovastatin, atorvastatin and simvastatin (MSKCC - Coenzyme Q10). No single statin stood out as safe from the effect.
How much CoQ10 should I take with a statin?
There is no agreed dose. Trials on statin muscle symptoms used 100-600 mg a day (Qu 2018 JAHA - CoQ10 statin symptoms), and the earlier review found no dose effect (Banach 2015 - CoQ10 statin myopathy). Cardio Clear 7 has 100 mg per capsule, the low end. Let your doctor set the plan.
Can CoQ10 replace a statin?
No. CoQ10 does not do a statin's job of lowering LDL cholesterol, and no trial shows it can. Statins slow the liver's cholesterol output (MedlinePlus - Statins). A supplement can only sit alongside prescribed care, with your doctor's approval.
How long does CoQ10 take to help statin aches?
The trials ran from 30 days to 3 months (Qu 2018 JAHA - CoQ10 statin symptoms), so no study gives a clear timeline. Some people notice nothing at all. Track how your muscles feel over a few weeks and share notes with your prescriber.
Can a statin cause muscle pain without damage?
Yes. Muscle damage from statins is uncommon, but aches can still happen, and many have other causes (MedlinePlus - Statins). A doctor may order a blood test or suggest another statin. Tell your prescriber instead of guessing.
